Abstract
The fact that most medical expenditures are paid by health insurance often blinds us to the actual costs of medical care. Despite the initiation of cost-saving measures known as "managed care" in the mid 1980s the cost of health insurance continues to rise and businesses either limit the coverage they provide their employees or give them less of a salary increase to make up the difference. Efficiency gains that helped contain costs initially have been exhausted; now cuts are impinging on the practice of medicine and the doctor/patient relationship.
