Abstract
Focusing on the challenge of treating chronic diseases, this program examines the human and economic costs of effectively managing diabetes, heart disease, and other long-term conditions that consume nearly 70 percent of all health care resources and affect nearly 100 million Americans. Viewers will encounter groundbreaking efforts taking place in two very different communities—the city of Los Angeles and Whatcom County in the state of Washington—that provide innovative ways of supporting patients who struggle with chronic conditions.
